I, acknowledge that the exercise classes given at FitStop Grand-Sault,
located at 245 Broadway Boulevard, Grand Falls, NB carries with it the
potential for personal injury. I hereby assume all of the risks of participating
in these classes. I certify that I am physically fit and have not been advised
not to participate in these type of classes by a qualified medical person.

In consideration of permitting me to participate in these exercise classes, I
hereby take action for myself, my executors, administrators, heirs, next of
kin, successors, and assigns to waive, release and discharge from any and
all liability for my death, disability, personal injury, or actions of any kind
which may hereafter accrue to me arising out of my participation in these
exercise classes, any persons acting as coaches and/or teachers of such
exercise classes, as well as the owners of Fitstop Grand-Sault.
